Pertussis Outbreak Among Adults at an Oil Refinery - Illinois, August-October 2002

石油精製所の成人従業員における百日咳のアウトブレイク－イリノイ，2002年8～10月

2002年9月16日、Crawford County Health Department(CCHD)はIllinois Department of Public Health(IDPH)にイリノイ州Crawford郡の石油精製所従業員のうち咳症状を呈する4例を報告した。8月14日、39歳の従業員が14日間続く咳を工場の健康管理部に報告した。同日、50歳の従業員監督者が3日間続く咳発作と咳漱失神のため同部を受診した。血清学的検査にて2例とも新種のBordetella pertussis感染が疑われ、CCHDに報告された。9月18日、IDPHとCCHDはサーベイランス活動と症例調査を開始した。その結果、8月1日－10月9日にこの発端となった監督者と接触のあった石油精製所従業員150人中15人(10%)が百日咳と診断された。12月13日現在、これら石油精製所に関連して発症した17例(確定4例、疑い13例)に加え、この石油精製所に疫学的には無関係の市民7例で百日咳の疑いがあることが確認された。24例全例がマクロライド系抗生物質による治療を受けた。また9月19日には、石油精製工場における百日咳患者17例と仕事上の接触があった従業員150人がアジスロマイシンの予防投与を受けた。10月9日以降、この石油精製所において百日咳患者は報告されていない。このアウトブレイク中に確認された百日咳24例中21例(88%)が20歳以上の成人であったことから、成人において感染力が強い疾患であることを認識し、全国的な診断および予防戦略を改善する必要がある。
