Exophiala Infection from Contaminated Injectable Steroids Prepared by a Compounding Pharmacy - United States, July-November 2002

調剤薬局にて調合された注入ステロイド剤によるExophiala感染－アメリカ，2002年7～11月

2002年7月5日、38.0℃の発熱、軽度項部硬直などによりノースカロライナ州のA病院に入院した77歳の女性がCSF検査にて髄膜炎が疑われる症状を呈した。抗生物質を投与したが奏効せず、18日のCSF再検査にて顕微鏡所見から酵母様成分が認められたため、アンホテリシンBの投与を開始した。24日、真菌培養によりExophiala(Wangiella)dermatitidis(E.dermatitidis)が同定され、ボリコナゾールおよびフルシトシン投与に変更したが、症状の改善は得られず入院51日目に死亡した。本症例は入院の100および35日前にペインクリニックAにて慢性腰痛に対し調剤薬局A(サウスカロライナ州)にて調合されたメチルプレドニゾロンの腰椎硬膜外注入を受けていた。また、8月14日、腰痛治療を受けていた61歳の女性が髄膜炎症状を呈しA病院に入院した。この症例も入院の84および34日前に同じ調剤薬局Aにて調合されたメチルプレドニゾロンの腰椎硬膜外注入を受けており、CSF検査にてE.dermatitidisが検出された。ボリコナゾールの投与を開始し、12月5日(治療開始70日後)に改善を得た。さらに3例(71、65、52歳女性)のE.dermatitidis髄膜炎または仙腸骨炎が報告され、いずれの症例も入院前に調剤薬局Aにて調合されたメチルプレドニゾロンの投与を受けていた。South Carolina Board of Pharmacy(SCBP)による調剤薬局Aの調査では、加圧滅菌器作業の不備が指摘され、American Society of Health-System Pharmacists(ASHP)による無菌室基準を満たしておらず、さらにCDCおよびFDAによる微生物培養検査では、未開封のメチルプレドニゾロン注射剤バイアルからE.dermatitidisが検出された。9月27日、SCBPはこの薬局に対し注射剤の販売中止を命ずるとともに製剤を回収させた。2州ではすでに投与を受けた症例に対して、発症に対する注意書を送付した。医師には引き続きメチルプレドニゾロンによるExophiala感染の危険性の認識が勧告され、疑われる症例の各州衛生局およびCDCへの報告が義務付けられた。
