Probable Variant Creutzfeldt-Jakob Disease in a U.S. Resident - Florida, 2002

アメリカ住民における新変異型クロイツェルトヤコブ病－フロリダ州，2002年

2002年4月18日、フロリダ保健局およびCDCに新変異型クロイツェルトヤコブ病(vCJD)症例(22歳男性、フロリダ在住)が報告された。この男性は2001年11月初旬、うつ症状と記憶障害、12月には筋肉運動および歩行変化、着替え困難、失禁を来し、2002年1月救急外来を受診した。頭部CTスキャンは正常であり、パニック発作と診断され抗不安剤が処方された。1月末、英国在住の母親が英国へ連れ帰り、その後3ヶ月の間に記憶障害や他の神経症状が進行し、入浴や着替えが不能となり、簡単な計算もできなくなった。混乱や幻覚が起こり、言語障害、運動緩慢、痙攣を来たし、National Prion Clinicを受診した。MRIによりvCJDを示唆する視床枕および視床後部領域のシグナル異常が認められ、扁桃腺生検およびウエスタンブロット分析によりプロテアーゼ抵抗性プリオン蛋白(PrP-res)が検出され、免疫組織検査により確定された。プリオン蛋白遺伝子分析では突然変異は認められず、コドン129においてメチオニンホモ接合性が認められた。3ヶ月間キナクリンが投与されたが2002年9月寝たきりとなり、極度の体重減少から栄養管が挿入され、家族とのコミュニケーションは不能となっている。本例は1979年英国にて出生(アメリカへは1992年に移住)、英国内においてウシ海綿状脳症の因子に暴露した可能性が高いと考えられる。
