Fatal Yellow Fever in a Traveler Returning from Amazonas, Brazil, 2002

ブラジル・アマゾナス旅行者における致死的黄熱病－2002年

蚊が媒介するウイルス性疾患の1つである黄熱病(YF)は、致死率が約20%にも及び、サハラ砂漠周辺地域や南アメリカ熱帯地域への旅行者での発症が報告されている。今回、ブラジルアマゾナス州マナウスへ旅行したテキサス州在住の47歳男性が、帰宅後YFを発症した(1996年以降3例目)。急激な下腹部痛、発熱(39.3℃)および強度の頭痛によりEDを受診し、血液検査では白血球減少症、貧血、血小板減少症および凝固異常、腎不全、肝不全が認められた。入院3日後にはショック症状、てんかん発作および静脈穿刺部位からの大量出血を来し、翌日死亡した。CDCによる血清検査では、YFウイルスIgM、IgG抗体は陰性であった。死亡した症例はアマゾン川への釣りツアーに参加していたが、YFワクチンやマラリア予防接種は受けていなかった。ツアー参加者15名のうち、WHOガイドラインに従ってYFワクチン接種を受けていたのは8名であり、11年前および到着5日前に各1名が接種、マラリア予防接種は3名のみであった。CDCではこれらの地域の旅行者に対し、旅行10日以上前にワクチン接種を受けるよう勧告している(詳細はhttp : //www.cdc.gov/travel)。
