Chagas Disease After Organ Transplantation - United States, 2001

2001年4月23日、急性シャーガス病(アメリカトリパノソーマ症)1例がCDCに報告された。この症例は3月5日に死体腎および膵移植を受けた37歳の女性であり、4月19日に発熱のため来院し、4月23日に末梢血塗抹標本にてT.cruziトリポマスティゴートが同定された。その後、同一ドナーから臓器提供を受けた2例(肝移植を受けた32歳女性、腎移植を受けた69歳女性)でもT.cruzi感染が確認された。これら3例における血液培養結果はT.cruzi陽性であった。ドナーは中央アメリカからの移民であり、おそらくT.cruziに感染していたと考えられるが、検査可能なドナー由来検体は入手できなかった。感染判明後、レシピエントはニフルティモクスによる治療を受けた。69歳女性は約4か月間治療を受け2002年3月の時点で再発のエビデンスはみられていないが、他の2例は死亡した。CDCと米国臓器移植協会は、T.cruzi感染の可能性のあるドナーのスクリーニングを推奨するかどうか、そして推奨する場合にはスクリーニングするドナーの選択法、スクリーニング法、スクリーニング結果が陽性の場合の対処について検討するつもりである。
